Helping Women Take Control of Their Lives and Move Out of Homelessness

New Generations Reunification Program – (A 24 months Transitional Family Housing Program)

Eligibility Criteria: Homeless unaccompanied adult Female (18 years or older); may have a history of substance abuse (sober for 3 months), domestic violence, minor mental health diagnosis (medication independent) and/or minor medical disabilities, must be displaced from minor child(ren) with the possibility of reunifying.

Total Occupancy Capacity: 15 Families

Referral Process: Must be referred by an outside social service agency, a telephone screening will be conducted with the referring agency as well as 2 face-to-face interviews with perspective resident(s).

Required Documentation:
• Verification of homelessness
• Verification of separation from minor child(ren)
• Any pertinent documentation as it relates to minor child(ren)
• Identification (birth certificate, social security card and picture identification)
• TB Test results
• Verification of sobriety
• Verification of mental health diagnosis (where applicable)
• Psychiatric Evaluation (where applicable)
• Psychosocial and/or social history
• Police clearance / Arrest record
