Helping Women Take Control of Their Lives and Move Out of Homelessness

New Horizons Supportive Housing Program-Permanent Housing for Families

Eligibility Criteria:
Homeless single adult women (18 years or older) who are recognized as head of household and meet the criteria by having one or a combination of the following: a history of substance abuse, domestic violence, minor mental health diagnosis, and/or diagnosis of HIV/AIDS.

Total Occupancy: 17 Families

Referral Process:
Must be referred by an outside social service agency, a telephone screening will be conducted with the referring agency as well as a face-to-face interview with perspective resident(s).

Required Documentation:

  • Verification of homelessness
  • Identification (birth certificate, social security card and picture identification)
  • TB Test results
  • Verification of sobriety (at least 3 months documented sobriety, where applicable)
  • Verification of mental health diagnosis (where applicable)
  • Psychiatric Evaluation (where applicable)
  • Psychosocial and/or social history
  • Police clearance
