New Expectations (NEXT) 24-months Transitional Housing Program

Eligibility Criteria:
Homeless unaccompanied adult (18 years or older) women; must have a history of substance abuse (sober for 30 days), may posses minor mental health diagnosis (medication compliant and independent) and/or minor medical disabilities.

Total Occupancy: 10 Pregnant Women

Referral Process:
Must be referred by an outside social service agency, a telephone screening will be conducted with the referring agency as well as a face-to-face interview with perspective resident(s).

• Verification of homelessness
• Verification of Pregnancy
• Identification (birth certificate, social security card and picture identification)
• TB Test results
• Verification of sobriety
• Verification of mental health diagnosis (where applicable)
• Psychiatric Evaluation (where applicable)
• Psychosocial and/or social history
• Police clearance
