Shelter Plus Care Permanent Housing Program (New Endeavors S+C)

Eligibility Criteria:
Homeless adult (18 years or older) women and/or families; must have a history of substance abuse (sober for 3 months), may posses minor mental health diagnosis (medication compliant) and/or minor medical disabilities.

Total Occupancy: 11 Individuals and Families

Referral Process:
Must be referred by The Community Partnership for the Prevention of Homelessness (TCP) and a face-to-face interview will be conducted with perspective resident(s) and New Endeavors by Women S+C Case Manager.

Required Documentation:

• Verification of homelessness
• Income Verification
• Any pertinent documentation as it relates to minor child(ren)
• Identification (birth certificate, social security card and picture identification)
• TB Test results
• Verification of sobriety
• Verification of mental health diagnosis (where applicable)
• Psychiatric Evaluation (where applicable)
• Psychosocial and/or social history
• Police clearance
